Letter to Families
This letter can be used as a basis for drafting a letter to give your family members to explain your BHD diagnosis. It may be helpful for your family to have your diagnosis in writing. You could also use this letter for family members who are not currently interested in pursuing a BHD diagnosis, but who may want to do so at a later date.
Portable Health Card
If you would like to carry your medical information with you, you can print this portable health card on normal printer paper. If you need unexpected treatment for collapsed lungs or kidney tumors, this card may be helpful to you. BHD lung collapses and kidney tumors are not always treated the same way as collapses and tumors that happen in the general population. Having your doctor’s contact information with you can help. Print this, cut it out and fold it in half, then in half again. It will be the same size as a credit card or business card. Below you will find a plain copy If you do not want to print the red color or do not have a color printer.
