Ozer et al. describe the case of a 49 year old male who presented in clinic with chest pain, shortness of breath and a dry cough. He had a medical history of repeat pneumothoraces and a family history of BHD syndrome. High resolution CT scans revealed the presence of multiple bilateral lung cysts 1-3 mm in diameter and the patient was subsequently diagnosed with BHD syndrome. The authors describe the anaesthetic management required to optimize oxygenation and CO2 excretionin the patient whilst minimizing air leak from bullae during surgical resection of large bullae.
